Dr Jeffrey Tan Jia Yeong

 

Dr Tan is a registered family physician in Singapore and has a Graduate Diploma in Family Medicine as well as a Graduate Diploma in Occupational Medicine. Dr Tan has worked in various disciplines such as Orthopaedics, General Surgery, General Medicine, Emergency Medicine, Geriatric Medicine and Radiology. He has also worked at polyclinics, Ang Mo Kio Family Medicine Clinic and in private general practice. It was his experience working in polyclinics that ignited his passion for family medicine.

Dr Tan brings with him extensive experience managing chronic diseases such as diabetes mellitus, hypertension, hyperlipidaemia and stable cardiorespiratory conditions. He firmly believes that a holistic person-centred approach is the best way to manage the multi-faceted issues that patients encounter these days.

Dr Kok Zi Yin

 

Dr Kok Zi Yin graduated with Bachelor of Medicine and Bachelor of Surgery (MBBS) from The University of Melbourne in November 2005. After graduation, Dr Kok started her family medicine training in various hospital departments such as General Medicine, Dermatology, Geriatric Medicine, Emergency Medicine, Paediatric Emergency Medicine, Neonatology, Obstetrics & Gynaecology, Orthopaedics, General Surgery and Psychiatry as well as in polyclinic.

She obtained her postgraduate Master’s degree in Family Medicine in 2012. As well as that, she completed her Diploma in Dermatology with Cardiff University in 2013.
Dr Kok has a deep passion and extensive experience in family medicine, having been a family physician with Singhealth Polyclinic, private general practice group, as well as Ang Mo Kio Family Medicine Clinic. She is able to converse in English, Mandarin, Malay, Cantonese and Hokkien.

Dr Loke Kam Weng

Dr Loke Kam Weng is a senior family physician and managing partner of EJ Tan Clinic & Surgery, Sims Drive Medical Clinic and Family Medicine Clinic Chinatown.

He graduated from National University of Singapore in 1990 and obtained his Master of Family Medicine in 1999 and his Fellowship conferred by College of Family Physician in 2007.

He is an adjunct lecturer and clinical tutor for both Yong Loo Lin School of Medicine and Duke-NUS Graduate Medical School and is also tutor and examiner for Master of Family Medicine and Graduate Diploma of Family Medicine.

He works part-time with the Primary and Community Care Division in the Ministry of Health and is the director of Joint Committee Family Medicine which oversees post-graduate family medicine training and examinations.
